Life Stage and Transition support helps participants navigate the significant changes that happen across a lifetime. Transitions can be unsettling, and the right planning makes them far easier to manage.
We support people through moving out of the family home, finishing school and moving into further study or work, changing service providers, adjusting to a new diagnosis, entering adulthood and planning for the future.
Our role is to help people understand their options, weigh them up, connect with the right mainstream and community services, and make decisions with confidence. We coordinate with families, schools and other providers so nothing falls through the cracks.

How We Deliver This Service
We map out each transition step by step, identify who needs to be involved and set realistic timeframes. Regular check-ins during and after the transition make sure supports are working and adjustments are made early if they're not.
