Community Nursing Care brings qualified nursing support directly to participants in their own home and community. For people in remote and rural areas, this removes the need to travel long distances for routine clinical care.
Our nursing services include medication management and administration, wound care, continence assessment and support, catheter and stoma care, diabetes management, health monitoring and training for support workers and carers in high-intensity supports.
Care is delivered by registered and enrolled nurses working to clear clinical care plans, in close communication with GPs, specialists and allied health professionals so that a participant's health care stays joined up.

How We Deliver This Service
A registered nurse completes an initial assessment and develops a clinical care plan tailored to the participant's health needs and NDIS goals. Care is reviewed regularly, documented carefully and adjusted in consultation with the participant's broader health team.
